REVIEW OF SUPPORTED LIVING RATES
The Oregon Office of Developmental Disabilities Services (ODDS) is in the process of  reviewing provider payment rates and structures for Supported Living. Burns & Associates, a division of Health Management Associates (HMA-Burns), is assisting ODDS in these efforts.

PROVIDER SURVEY
A key factor in this review is a provider survey to collect data regarding Supported Living programs:

• The Excel-based survey can be accessed HERE
• The survey instructions can be accessed HERE
• A webinar in which B&A walks through the survey can be accessed HERE

Information collected through the survey will be utilized only for the purpose of this rate study and vendors’ individual responses will not be released.
Completed surveys are due by Friday, December 24 and should be submitted to  [email protected].
